Vaccination List

All vaccines are fully covered for students using the United HealthCare Student Plan.

Required for entry to ÃÛÌÒapp

Hepatitis B
  • The first dose may be given anytime
  • The second dose may be given 1–2 months after the first dose
  • The third dose may be given 4–6 months after first dose
  • The 0.5cc dose is given to individuals 19 and younger
  • The 1cc dose is given to individuals 20 and older
  • A Hepatitis B titer with a positive result fulfills this requirement

MMR (Measles Mumps Rubella)
  • Ìý2 doses of MMR vaccine are required for children
    • The first dose at 12–15 months of age
    • The second dose at 4–6 years of age
  • Adults who have not completed this series as a child need two doses of MMR vaccine
    • Must be at least four weeks apart
    • A positive blood titer for Measles (Rubeola), Mumps and Rubella also fulfills requirements

Meningitis A, C, Y, W-135Ìý
  • MCV4 is required for all unvaccinated teens through 23 yrs
  • If previously vaccinated, a booster dose is required at age 16 yrs or older
  • Additional booster must be given every five years if risk continues

PolioÌý
  • If injectable polio vaccine (IPV) or oral polio vaccine (OPV) was given as a child in a four-dose series, this fulfills the requirement
  • If not, three doses of IPV are required (first dose at anytime, second dose 1–2 months later, third dose 6–12 months after second dose)

Tetanus Diphtheria PertussisÌý
  • At least three doses of any tetanus and diphtheria vaccine (DTP, DTap, DT, Td or Tdap) are required
  • One of Tdap is required at age 11–18 yrs
  • Booster doses of Td or Tdap required every 10 years

Tuberculin testing
  • A TB self-screening form must be completed. Once submitted, students will be notified if a TB test is needed
  • A TB test is recommended three months after travelling to high risk countries
  • A TB test must be read within 48–72 hours after adminstration
  • Any TB blood tests with a negative result fulfills the requirement in place of a TB skin test

Varicella
  • 2 doses required for those who have never had the chicken pox disease. First dose between 12–15 months of age, second dose between 4–6 yrs and at least three months after the first dose, up to the age of 13.
  • For people 13 yrs or older, 2 doses (28 days apart or more) are required
  • A varicella titer with a positive result also fulfills this requirement

Other recommended vaccines

Hepatitis AÌý
  • This vaccine series may be started whenever a person wishes to reduce their risk of the disease, or for travelers, the vaccine should be started at least one month prior to traveling for the best protection. A second dose should be given at least six months later
  • Two doses of the vaccine are required for lasting protection. First dose 12 months to 23 months of age. The second dose 6-18 months later

HPV-GardasilÌý
  • This vaccine is recommended for males and females ages 9–26 yrs
  • Most adolescents ages 9–14 yrs should receive HPV vaccine as a two-dose series, with the doses separated by 6–12 months
  • If the first dose is given after the age of 15, it is a three-dose series with the second and third doses given 2–6 months after the first

InfluenzaÌý
  • All people six months of age and older should receive the flu vaccine annually
  • It is available in September and throughout the flu season

Typhoid (injectible or oral)Ìý
  • Inactivated vaccine required at least two weeks prior to traveling to areas of risk (A booster is required every two years)
  • Live (oral): four doses given at least one week before traveling abroad (A booster is required every five years)

Yellow FeverÌý
  • Anyone nine months of age or older, traveling to or living in a country that requires yellow fever vaccination, is required to get this vaccine
  • Anyone nine months of age or older traveling to or living in a country that does not require yellow fever vaccination but is located in an area where the risk of yellow fever is known to exist, should receive this vaccine
  • This vaccination is valid for a lifetime, with proof

  • Vaccines available for all those that are 5 years and above.
  • Pfizer-BioNTech
    • Primary Series
    • 2 Doses
    • Given 3 weeks (21 days) apart
    • Everyone 18 and above should get a booster dose of either Pfizer-BioNTech or Moderna (COVID-19 vaccines) 5 months after the last dose in their primary series. Teens 12-17 years old should get a Pfizer-BioNTech COVID-19 vaccine booster 5 months after the last dose in their primary series.

    • Primary Series
    • 2 Doses
    • Given 4 weeks (28 days) apart
    • Everyone ages 18 and older should get a booster dose of either Pfizer-BioNTech or Moderna 5 months (COVID-19 vaccines) after the last dose in their primary series.
  • Johnson & Johnson’s Janssen (J&J)
    • Primary Series
    • 1 Doses
    • Everyone ages 18 and older should get a booster dose of either Pfizer-BioNTech or Moderna (COVID-19 vaccines) at least 2 months after the first dose of J&J/Janssen COVID-19 vaccine. You may get J&J/Janssen in some situations.

Meningitis BÌý
  • This vaccine is given as a two-dose (or occasionally a three-dose) series recommended for individuals >10yrs of age at increased risk for Men B disease: (1) persons with compliment deficiency, including compliment inhibitor use, (2) persons with functional or anatomical asplenia, (3) persons such as microbiologists and others exposed to N. meningitidis.
  • There are two types of vaccine: Bexsero and Trumenba. They are not interchangeable, so be sure to indicate which vaccine you have received when you send in your records.
  • Based on the latest CDC recommendations, this is no longer a mandated vaccine for ÃÛÌÒapp. Please discuss the risk of the disease and weigh the risk and benefit of the vaccine with your provider.
